This form must be completed by a child's parent or guardian in order for a child to take any medication while at school. This includes over-the-counter medication such as Aspirin and even cough drops as well as any prescribed medicines such as antibiotics or medication for ADHD. There can be absolutely NO exceptions to this rule.

This form is to give your child permission to self-medicate. Usually, it is used for students who may need to carry their asthma inhaler with them rather than leaving it in the office.
